Case Manager, Wiggins Campus
- QUALIFICATIONS
The Case Manager for Baptist Children’s Village & Family Ministries (“BCVFM”) must be a mature Christian adult whose personal integrity, character, and reputation reflect the standards and ethics expected of BCVFM staff. This individual must hold a bachelor’s degree in a social or behavioral science field and have a minimum of two years of case management experience. The Case Manager must demonstrate effective communication skills, sound judgment, assertiveness, and the ability to work collaboratively with residents, families, referral sources, and BCVFM staff. This position requires schedule flexibility and availability during times when residents are on campus or when families are accessible.
- DUTIES & RESPONSIBILITIES
- Conduct new admissions, transfers, and discharges.
- Serve as a member of the Cottage Team and Program Team.
- Present and review the IPOC with residents and their families.
- Maintain responsibility for accurate documentation of case management activities and all clinical information required in the case file.
- Arrange visitation for residents within the caseload.
- Represent the interests of the child and the agency in all school-related matters.
- Serve as liaison for residents receiving external or contracted psychiatric or mental health services.
- Represent BCVFM at reviews conducted by public authorities for residents in the caseload.
- Provide CQI data as outlined in the Program Manual.
- Assist with the implementation of TBRI.
- Provide aftercare services to families as needed.
- Perform other duties as assigned by the supervisor.
